Upgrade Modal Popup w/ Pricing Table

Hey everyone! Here's a popup plan upgrade modal/lightbox with a pricing table inside of it. This would typically be used when a member is on a free plan, but they tried to access something which is only for paying members. Don't forget to clone the code in the sitewide <head> so this works!
