How to Add a Feedback Poll / Survey Widget to Your Webflow Site with Data Tables

Let logged-in members submit feedback with a rating and optional comment. Responses are saved to a Memberstack Data Table. The widget automatically hides if the member has already responded, and opens on page load for members who haven't.
